Why Become a Host or Companion Model Home Provider |

Whether you are single or married, a professional, a retiree or a homemaker, you can become a Host or Companion Model Home provider for Innovations if you believe that a safe, caring, and nurturing environment is essential to the health and well-being of everyone.
Becoming a Host or Companion Model Home provider is not a passive act of opening one’s home, or moving in to one’s home, to provide food, clothing, and shelter. It is a proactive statement of advocacy and nurturing.

Host and Companion Home Provider Duties |
|
- Provides a supportive and safe home for an individual with a developmental disability (consumer).
- Provides training and/or supports consumer in team-identified objectives that include health and safety, leisure activities, social skills and activities, independent living skills, language skills, and money management.
- Attends and provides input into consumer's annual Individual Plan (IP) meeting.
- Maintains records showing the consumer's progress toward the (IP) goals and objectives and provides reports as required by the social worker/case manager.
- Maintains knowledge of state and Imagine! rules and regulations for HCB-DD programs.
- Coordinates communication and serves as liaison regarding the consumer with the Imagine! staff, contract personnel, family, and other agencies.
- Completes training in First Aid, CPR, Medication Administration and other training as required by regulation and Imagine! policy.
- May perform other duties related to the training of the consumer as determined through the IP.
- Assists the consumer with shopping, bill paying, other financial management as requested.

Requirements |

- Complete a Host Home Application or a Companion Home Application.
Please note: In order to best serve our consumers, generally Innovations only contracts with Host Home and Companion Home Providers who reside within an area that is no more than a 60 minute drive from our Lafayette, CO. office.
- Successfully pass background checks and reference checks.
- Maintain a valid Colorado Driver's License and safe driving records (for consumers who require transportation).
- Provide a home that meets HUD and Imagine! standards.
- Maintain personal liability insurance and automobile liability insurance at the required levels.
- Complete an interview at Imagine!.
- Successfully complete required training.
- Demonstrate writing skills adequate to complete necessary documentation of consumer progress.
- Demonstrate knowledge of basic training techniques, good interpersonal skills, home management skills, and the ability to learn specific support needs of the individual to be served.
