Garden Place PCA
A family home located in Longmont, Colorado. This spacious home features a live-in staff member, additional daily support staff, and three individuals with disabilities. Garden Place offers a strong devotion towards learning new skills and finding more effective methods to achieve goals. This home strengthens opportunities for community access and inclusion.
Satellite Apartment Program
Innovations takes pride in supporting individuals to be as independent as possible. With this goal in mind, Innovations offers an independent apartment program that supports individuals to live in apartments throughout Boulder and Broomfield counties while still providing support for skill development as preferred and needed. Individuals have access to 24-hour support from staff, including staff support for money management, shopping, medical appointments, and social events as requested.
Individuals who are successful living in an independent apartment setting have strong, well established skill sets including:
Does not require 24-hour on-site supervision.
Does not require physical assistance with personal hygiene on a daily basis.
Does not require assistance with daily medical or nursing type intervention. Participant must be able to take all medications with minimal support.
Recent physical evaluation and description of any medications required.
Can independently evacuate in case of fire/emergency.
Willing to make and receive phone calls and communicate with staff within preferred methods of communication.
